How to Send the Release Form?

We cannot process your order without the signed release form. You have two easy options to send it:

1. Upload the Form Online

   Click this link to securely upload the signed release form.

2. Email the Form

   Send the signed release form to [e-mail].
   Please include your Order ID in the subject line and attach the files in the email.
